Please read the following exercise advice carefully:
Work at a low level on your first session and concentrate on learning to do the exercises properly. On each session you will be able to work a little harder. Be sure to limit yourself to a pace where you can still talk comfortably. Should you suffer any injury, illness or condition in the future, please tell us by completing this form again.
It is recommended that all Males > 35 and Females > 45 should have a Medical Assessment including an Exercise E.C.G and Cholesterol / Lipid Count. Statement
I recognise that a Revolution trainer is not able to provide me with medical advice with regards to my medical fitness and that this information is used as a guideline to the limitations of my ability to exercise.
I have answered the questions to the best of my ability, understand the advice above and have read and understood Revolution PTS’s terms and conditions below. If you do not have a copy of Revolution PTS’s terms and conditions please inform a Revolution representative to request one.
TERMS & CONDITIONS
- Parties & Governing Law
(a) These Terms and Conditions (in which are incorporated these terms and conditions and the “session validation sheet” to be completed pursuant to paragraph 4 below) are made between Revolution Personal Training Studios Ltd
(“the Company”) and the individual client. (b) These Terms and Conditions shall be governed by and construed in all respects in accordance with the laws of England and Wales.
- Service & Fees
(a) The Client engages the Company to provide Personal Training / Massage Therapy (“PT”) being a focused session of exercise (the minimum duration of which is specified) led by a qualified instructor, bespoke for the wants and needs of
the client (“the Services”).
(b) The Company shall be responsible for providing all equipment deemed necessary by the Company in order to provide the Services.
(c) Nothing in this Agreement and nothing in any statements (whether written or oral) made by the Company, its employees or agents, should be interpreted or construed as being in any way a promise or guarantee as to progress or results that the Client may obtain from the Services or his/her fitness programme.
(d) The Client may not sell, assign or transfer his/her rights to the Services being provided hereunder to any other party without the Company’s prior written consent.
(e) Fees payable pursuant to this Agreement are not refundable. The Client shall make payment of all fees to the company via the reception at the Club. The Club’s reception will issue to the Client a receipt, which will constitute proof of payment.
(f) The client shall not be entitled to use the Services hereunder if he/she is in arrears with his/her split payment.
- Physical Condition of Client and Limitation of Liability
(a) The Company has conducted a health assessment in relation to the Client (a copy of which is available to the Client on demand). The Client acknowledges that the Company’s employees and agents are not medically trained and it is the Client’s responsibility to satisfy himself/herself as to his/her medical condition through his/her own doctor before embarking on his/her sessions.
(b) The Client confirms that, save as disclosed by him/her on his/her health assessment form, he/she is in good physical condition, that he/she is capable of engaging in active or passive exercise and that such exercise would not be detrimental to his/her health, safety, comfort or physical condition.
(c) Neither the Company nor any employee or agent of the Company shall be liable for any death or injury occurring during the prevision of the Services except to the extent that such death or personal injury arises from any negligent act or omission of the Company or any employee or agent of the Company.
- Booking System
(a) The “booking system” is part of this Agreement.
(b) Simultaneously with the Client purchasing the Services, the Client’s pt session count will be recorded and tracked on “the booking system”.
(c) At the end of each PT session, the Company shall action the booked PT session reducing the Client’s block of sessions by 1.
(d) Where a client has not attended a PT session and has not given notice of cancellation pursuat to paragraph 5(a) below the Company shall action that session on “the booking system” indicating that the PT session has been used.
(e) In the absence of manifest error, “the booking system” shall be conclusive evidence as to the number of PT sessions booked, taken and their respective dates when used.
- Remote Personal Training
(a) When training or handling your training equipment at home, please take extra care that all your equipment and the area that you are going to train in are maintained and safe for you to exercise in before each session.
(b) When you or anything that you are using for your remote personal training session is either picked up, put down or otherwise manoeuvred, please do so safely to protect yourself from injury and your property/training area from damage.
(c) Any damage caused to your home or training area by either yourself or your training equipment is not the responsibility of Revolution Personal Training Studios.
- Cancellation
(a) If a booked personal training session is cancelled by the client for any reason, a period of no less than 24 hours must be given to their assigned personal trainer otherwise the client will be charged for the session which in turn will reduce their sessions remaining count by 1.
Note: If at any point during your personal training session (outside or in the studio), during your dieting or eating plan or during your remote personal training session you feel unwell, in pain, dizzy, faint or if anything else out of the ordinary or just doesn’t seem right, then listen to your body, stop and let your personal trainer know immediately! If you feel this way before a training session then you should let your personal trainer know beforehand.
